Evaluate continuity in Medicare Advantage navigation
Prioritize clinical context, understandable next steps, verified network options, and completed handoffs. Evaluate how the workflow supports members who need help using digital tools, and ensure responsibility is clear when a recommended option cannot be completed.

Define the journey, not just the front door
Choose a specific member entry channel and receiving care team. Write down what the member hears or sees, who reviews the information, and what happens after a choice is confirmed. Identify where an existing care manager needs to remain involved.
Plan for assisted use
Test readability, simple instructions, and staff-assisted workflows during evaluation. Determine how authorized representatives are handled in the configured product. Do not assume every member can independently complete a digital pathway or that proxy permissions exist without demonstration.
Verify plan-specific availability
Check networks, benefit scope, and destination access for the selected population. Transport and other supporting services depend on implementation and eligibility. A broad platform description is not evidence that a particular member has a covered benefit.
Set a balanced pilot scorecard
Track completion, response to unsuccessful handoffs, member experience, and relevant subsequent utilization. Claims-based cost analysis requires an appropriate comparison and follow-up period. Navigation activity should not be presented as a demonstrated improvement in quality ratings or clinical outcomes.
